From my working experience， I found it&#8217;s a great preparation for the world outside schools. I understood not only the value of money but also the way of getting along with others. What&#8217;s more， I have got some working and social experience. It&#8217;s good for my future work. In a word， I&#8217;ve had a wonderful vacation. </span></p>

<p>Epilepsy is a brain disorder involving recurrent seizures.</p>
<p>Causes, incidence, and risk factors</p>
<p>Epilepsy is a disorder involving repeated seizures of any type. Seizures (&#8221;fits&#8221;) are episodes of disturbed brain function that cause changes in attention and/or behavior. They are caused by abnormal electrical excitation in the brain.</p>
<p>Sometimes, seizures are related to a temporary condition, such as exposure to drugs, withdrawal from certain drugs, or abnormal levels of sodium or glucose in the blood. In such cases, repeated seizures may not recur once the underlying problem is corrected.</p>
<p>In other cases, injury to the brain (e.g., stroke or head injury) causes brain tissue to be abnormally excitable. In some people, an inherited abnormality affects nerve cells in the brain, which leads to seizures. In some cases, no cause can be identified.</p>
<p>Some of the more common causes of seizures include:</p>
<p><span><span id="more-317"></span></span></p>
<p>* Idiopathic (no identifiable cause)</p>
<p>o usually begin between age 5 and 20</p>
<p>o can occur at any age</p>
<p>o no other neurologic abnormalities present</p>
<p>o often a family history of epilepsy or seizures</p>
<p>* Developmental or genetic conditions present at birth, or injuries near birth —— in this case, the seizures usually begin in infancy or early childhood</p>
<p>* Metabolic abnormalities</p>
<p>o may affect people of any age</p>
<p>o diabetes complications</p>
<p>o electrolyte imbalances</p>
<p>o kidney failure, uremia (toxic accumulation of wastes)</p>
<p>o nutritional deficiencies</p>
<p>o phenylketonuria (PKU) —— can cause seizures in infants</p>
<p>o other metabolic diseases, such as inborn error of metabolism</p>
<p>o use of cocaine, amphetamines, alcohol, or certain other recreational drugs</p>
<p>o withdrawal from alcohol</p>
<p>o withdrawal from drugs, particularly barbiturates and benzodiazepines</p>
<p>* Brain injury</p>
<p>o may affect any age, but most common in young adults</p>
<p>o most likely to occur if the brain membranes are damaged</p>
<p>o seizures usually begin within 2 years after the injury</p>
<p>o early seizures (within 2 weeks of injury) —— do not necessarily indicate that chronic (ongoing) seizures (epilepsy) will develop</p>
<p>* Tumors and brain lesions that occupy space (such as hematomas)</p>
<p>o may affect any age, more common after age 30</p>
<p>o partial (focal) seizures most common initially</p>
<p>o may progress to generalized tonic-clonic seizures</p>
<p>* Disorders affecting the blood vessels (such as stroke and TIA)</p>
<p>o most common cause of seizures after age 60</p>
<p>* Degenerative disorders (senile dementia Alzheimer type, or similar organic brain syndromes)</p>
<p>o mostly affect older people</p>
<p>* Infections</p>
<p>o may affect people of all ages</p>
<p>o may be a reversible cause of seizures</p>
<p>o brain infections like meningitis and encephalitis can produce seizures</p>
<p>o brain abscess</p>
<p>o acute severe infections of any part of the body</p>
<p>o chronic infections (such as neurosyphilis)</p>
<p>o complications of AIDS or other immune disorders</p>
<p>Seizure disorders affect about 0.5% of the population. Approximately 1.5-5.0% of the population may have a seizure in their lifetime. Epilepsy can affect people of any age.</p>
<p>Risk factors include a family history of epilepsy, head injury, or other condition that causes damage to the brain.</p>
<p>The following factors may present a risk for worsening of seizures in a person with a previously well-controlled seizure disorder:</p>
<p>* Pregnancy</p>
<p>* Lack of sleep</p>
<p>* Skipping doses of epilepsy medications</p>
<p>* Use of alcohol or other recreational drugs</p>
<p>* Certain prescribed medications</p>
<p>* Illness</p>
<p>Symptoms</p>
<p>The severity of symptoms can vary greatly from simple staring spells to loss of consciousness and violent convulsions. For many patients, the event is stereotyped (the same thing over and over) while some patients have many different types of seizures that cause different symptoms each time.</p>
<p>The type of seizure a person experiences depends on a variety of factors, such as the part of the brain affected, the cause, and individual response.</p>
<p>An aura consisting of a strange sensation (such as tingling, smell, or emotional changes) occurs in some people prior to each seizure. Seizures may occur repeatedly without explanation.</p>
<p>SYMPTOMS OF GENERALIZED SEIZURES</p>
<p>Generalized seizures affect all or most of the brain. They include petit mal and grand mal seizures.</p>
<p>Petit mal seizures:</p>
<p>* Minimal or no movements (usually, except for &#8220;eye blinking&#8221;) —— may appear like a blank stare</p>
<p>* Brief sudden loss of awareness or conscious activity —— may only last seconds</p>
<p>* Recurs many times</p>
<p>* Occurs most often during childhood</p>
<p>* Decreased learning (child often thought to be day-dreaming)</p>
<p>Tonic-clonic (grand mal) seizures:</p>
<p>* Whole body, violent muscle contractions</p>
<p>* Rigid and stiff</p>
<p>* Affects a major portion of the body</p>
<p>* Loss of consciousness</p>
<p>* Breathing stops temporarily, then &#8220;sighing&#8221;</p>
<p>* Incontinence of urine</p>
<p>* Tongue or cheek biting</p>
<p>* Confusion following the seizure</p>
<p>* Weakness following the seizure (Todd&#8217;s paralysis)</p>
<p>SYMPTOMS OF PARTIAL SEIZURES</p>
<p>Partial seizures affect only a portion of the brain.</p>
<p>Simple partial (focal) seizures:</p>
<p>* Muscle contractions of a specific body part</p>
<p>* Abnormal sensations</p>
<p>* May have nausea, sweating, skin flushing and dilated pupils</p>
<p>* May have other focal (localized) symptoms</p>
<p>Partial complex seizures:</p>
<p>* Automatism (automatic performance of complex behaviors without conscious awareness)</p>
<p>* Abnormal sensations</p>
<p>* May have nausea, sweating, skin flushing and dilated pupils</p>
<p>* May have other focal (localized) symptoms</p>
<p>* Recalled or inappropriate emotions</p>
<p>* Changes in personality or alertness</p>
<p>* May or may not lose consciousness</p>
<p>* Olfactory (smell) or gustatory (taste) hallucinations or impairments —— if the epilepsy is focused in the temporal lobe of the brain.</p>
<p>Signs and tests</p>
<p>The diagnosis of epilepsy and seizure disorders requires a history of recurrent seizures of any type. A physical examination (including a detailed neuromuscular examination) may be normal, or it may show abnormal brain function related to specific areas of the brain.</p>
<p>An electroencephalograph (EEG), a reading of the electrical activity in the brain, may confirm the presence of various types of seizures. It may, in some cases, indicate the location of the lesion causing the seizure. EEGs can often be normal in between seizures, so it may be necessary to do prolonged EEG monitoring.</p>
<p>Tests may include various blood tests to rule out other temporary and reversible causes of seizures, including:</p>
<p>* A CBC</p>
<p>* A blood chemistry, blood glucose</p>
<p>* Liver function tests</p>
<p>* Kidney function tests</p>
<p>* Tests for infectious diseases</p>
<p>* A CSF (cerebrospinal fluid) analysis</p>
<p>Tests for the cause and location of the problem may include procedures such as:</p>
<p>* A head CT or MRI scan</p>
<p>* A lumbar puncture (spinal tap)</p>
<p>Disorders that may cause symptoms resembling seizures include transient ischemic attacks (TIAs), rage or panic attacks, and other disorders that cause loss of consciousness.</p>
<p>Treatment</p>
<p>For treatment of seizures, please see Seizures - first aid.</p>
<p>If an underlying cause for recurrent seizures (such as infection) has been identified, the cause should be treated, and this may stop the occurrence of further seizures. This may include surgical repair of tumors or brain lesions, or other treatments.</p>
<p>Oral anti-convulsants may reduce the number of future seizures. Response is individual, and the medication used and dosage may have to be adjusted repeatedly. The type of medicine used depends on the seizure type, as some seizure types respond well to one medication and may respond poorly (or even be made worse) by others.</p>
<p>The need for follow-up depends on the seizure type and medications used. Some medications need to be monitored for side effects and blood levels.</p>
<p>For some patients, the use of several medications may still be inadequate. This is called refractory epilepsy. Some such people may benefit from brain surgery to remove the abnormal brain cells that are causing the seizures. For others, a vagal nerve stimulator is implanted in the chest, which can help reduce the number of seizures.</p>
<p>Patients should wear medical alert jewelry so that prompt medical treatment can be obtained if a seizure occurs.</p>
<p>Support Groups</p>
<p>The stress caused by having seizures (or being a caretaker of someone with seizures) can often be helped by joining a support group. In these groups, members share common experiences and problems. See epilepsy - support group.</p>
<p>Expectations (prognosis)</p>
<p>Epilepsy may be a chronic, lifelong condition. In some cases, the need for medications may be reduced or eliminated over time. Certain types of childhood epilepsy resolve or improve with age. A seizure-free period of 4 years may indicate that reduction or elimination of medications is possible.</p>
<p>Death or permanent brain damage from seizures is rare, but can occur if the seizure is prolonged or 2 or more seizures occur close together (status epilepticus). Death or brain damage are most often caused by prolonged lack of breathing and resultant death of brain tissue from lack of oxygen. There are some cases of sudden, unexplained death in patients with epilepsy.</p>
<p>Serious injury can occur if a seizure occurs during driving or when operating dangerous equipment, so these activities may be restricted for people with poorly controlled seizure disorders.</p>
<p>Infrequent seizures may not severely restrict the person&#8217;s lifestyle. Work, school, and recreation do not necessarily need to be restricted.</p>
<p>Complications</p>
<p>* Prolonged seizures or numerous seizures without complete recovery between them (status epilepticus)</p>
<p>* Injury from falls, bumps, or self-inflicted bites</p>
<p>* Injury from having a seizure while driving or operating machinery</p>
<p>* Inhaling fluid into the lungs and subsequent aspiration pneumonia</p>
<p>* Permanent brain damage (stroke or other damage)</p>
<p>* Difficulty with learning</p>
<p>* Side effects of medications</p>
<p>* Many anti-epileptic medications cause birth defects —— women wishing to become pregnant should alert their doctor in advance in order to adjust medications</p>
<p>Calling your health care provider</p>
<p>Call your local emergency number (911) if this the first time a person has had a seizure or a seizure is occuring in someone without a medical ID bracelet (instructions explaining what to do). In the case of someone who has had seizures before, call the ambulance for any of these emergency situations:</p>
<p>* This is a longer seizure than the person normally has, or an unusual number of seizures for the person</p>
<p>* Repeated seizures over a few minutes</p>
<p>* Repeated seizures where consciousness or normal behavior is not regained between them (status epilepticus)</p>
<p>Call your health care provider if any new symptoms occur, including possible side effects of medications (drowsiness, restlessness, confusion, sedation, or others), nausea/vomiting, rash, loss of hair, tremors or abnormal movements, or problems with coordination.</p>
<p>Prevention</p>
<p>Generally, there is no known way to prevent epilepsy. However, adequate diet and sleep, and abstinence from drugs and alcohol, may decrease the likelihood of precipitating a seizure in people with epilepsy.</p>
<p>Reduce the risk of head injury by wearing helmets during risky activities; this can help lessen the chance of developing epilepsy.</p>

Experts said that the case could mark a landmark in the treatment of cancer.<br />
It raises hopes of a possible new way of fighting the disease, which claims 150,000 lives in Britain every year.<br />
Ed Yong, health information manager at Cancer Research UK, said: &#8220;It&#8217;s very exciting to see a cancer patient being successfully treated using immune cells cloned from his own body. While it&#8217;s always good news when anyone with cancer gets the all clear, this treatment will need to be tested in large clinical trials to work out how widely it could be used.&#8221;<br />
However, the treatment could prove extremely expensive and scientists say that more research is needed to prove its effectiveness.<br />
Genetically altered white blood cells have been used before to treat cancer patients but this is the first study to show that simply growing vast numbers of the few immune cells in the body to attack a cancer can be safe and effective.<br />

Dr Cassian Yee, who led the team at the Fred Hutchinson Cancer Research Centre in Seattle, said: &#8220;For this patient we were successful, but we would need to confirm the effectiveness of therapy in a larger study.&#8221;<br />
The work raises hopes that this approach could not only offer a more effective treatment for skin cancer, or melanoma, which kills around 2,000 people in Britain alone, but be applied to other cancers too.<br />
The patient was one of nine with metastatic melanoma, that is skin cancer that has spread, who were being treated in a recently completed clinical trial to test bigger and bigger doses of their own white blood cells.<br />
Larger, more elaborate, trials are now under way.<br />
Almost 9,000 new cases of melanoma, the most serious form of skin cancer, are diagnosed every year in Britain, and nearly 2,000 patients die from the disease.<br />
Prof Peter Johnson, Cancer Research UK&#8217;s chief clinician, said: &#8220;This is another interesting demonstration of the huge power of the immune system to fight some types of cancer.<br />
&#8220;Although the technique is complex and difficult to use for all but a few patients, the principle that someone&#8217;s own immune cells can be expanded and made to work in this way is very encouraging for the work that Cancer Research UK and others are carrying out.&#8221;<br />
Immunotherapy, in which a patients own immune cells are used to treat cancer, is a growing area of research that aims to develop less-toxic treatments than standard chemotherapy and radiation.<br />
Because cancer occurs when the body&#8217;s own cells grow out of control, the immune system only responds weakly.<br />
The ability of the body&#8217;s own defences to tackle cancer in this case is all the more remarkable because most deadly feature of the disease is its ability to colonise other parts of the body, when it becomes much more difficult to treat.<br />
A dramatic example of immunotherapy was reported two years ago by one pioneer of the field, Dr Steven Rosenberg of the US National Cancer Institute, who eradicated cancer from two dying men using genetically modified versions of their own cells.<br />
Both Mark Origer and &#8220;Thomas M&#8221; were suffering from advanced melanoma but the hope is that such methods could be customised to attack other common cancers, notably breast, colon and lung.<br />
Dr Rosenberg told The Daily Telegraph the new work is an &#8220;interesting study that helps to confirm the effectiveness of cell transfer immunotherapy for treating cancer patients. We have now treated 93 patients with metastatic melanoma using their own anti-tumour cells with response rates up to 72 per cent.<br />

nurses are of particular importance for patients with cancer of the oesophagus and other upper gastrointestinal cancers. This hypothesis is based on circumstances that are typical for these<br />
tumours: (1) The prognosis is generally poor. In oesophageal and pancreatic cancer it is worse than any other tumour (Berrino et al., 1999). (2) The diagnostic procedure is complex. It is crucial to establish the tumour stage reliably, since this<br />
determines the choice of treatment. (3) The treatment is demanding. Extensive surgery is usually required to offer any chances of cure (Enzinger and Mayer, 2003). This surgical treatment, especially in oesophageal cancer, carries a<br />
substantial risk of severe complications (Viklund et<br />
al., 2006) and of long-term impairment of the quality of life (Viklund et al., 2005). (4) Patient management is concentrated to a few hospitals. Since it has been proven that high-volume surgery for oesophageal and pancreatic cancer substantially improves the survival (Birkmeyer et al., 2002; Birkmeyer et al., 2003), these patients are now being concentrated to fewer hospitals. In view of the complex, multidisciplinary and increasingly centralised care pathway of these patients, and since these patients in general often suffer considerably both from their tumour symptoms and from psychosocial problems involved in the awareness of their newly diagnosed cancer (Winterlinget al., 2004), it is important to ascertain optimal<br />
care, qualified coordination and continuous<br />
support. To improve the care of the increasing number of patients with cancer of the upper gastrointestinal tract at our department, a specialist nurse was employed. This nurse was appointed as the contact person for support of patients with upper gastrointestinal cancer, and was given the responsibility<br />
to plan their care pathway. Some investigations that addressed specialist nurses in the treatment of cancer have shown promising results (Moore, 2004; Amir et al., 2004; Koinberg et al., 2004; Mooreet al., 2002; Smith and Waltman, 1994; Rawl et al.,2002; Cox and Wilson, 2003; Loftus and Weston, 2001; Booth et al., 2005), but this has not been evaluated with regard to patients with upper gastrointestinal cancers.</p>

Divide the dough into 5 portions, roll out into a pancake about 6     <br />inches in diameter. Heat a frying pan, add a litte oil, fry pancake until opaque. Remove from heat.     <br />3. Beat egg, add some chopped scallions and salt, mix well and fry     <br />the egg mixture with some oil, cover with a pancake,then flip over     <br />and roll up, cut into several pieces,then serve.</p>

<p>I replaced the headlights in my car with strobe lights, so it looks like I&#8217;m the only one moving.</p>
<p>I play the harmonica. The only way I can play is if I get my car going really fast, and stick it out the window. I&#8217;ve been arrested three times for practicing.</p>
<p>I put a new engine in my car, but forgot to take the old one out. Now my car goes 500 miles per hour. The harmonica sounds *amazing*.</p>
